On the main screen, the sample format (left screen) does not match my pattern.  I don't know what I did to change things.

Even your saved formats have no effect.  It started dropping the date of the episode, then everything changed.

I would like to send a screenshot but your system does not allow this, that is all I can do.

My pattern:  %N (%Y) - S%S2E%E2 (%D) %T [%R,%V]

The displayed format in the setting is correct.  (can't copy it for here)

The output of the sample display ends up like this: (Can't copy it but have to enter it here)

  Father Ted - S 1, E ! - Good Luck, Father Ted

Renaming used this format, not mine.

If I could give you screenshots it would be much easier to debug.

thanks for a great program, It has saved me hours.

Ehm ... that is an interesting problem that I cannot reproduce. Your format string works fine on my computer;

And there are no options to remove zero's.

Do you have any special character replacements set maybe?
This is the default list of characters that will be replaced:

I don't know what happened.  It was working fine.

Are there any settings that might affect this?

This is weird ...
You already established it; but man, something odd is going on for sure.
The new names do not even get close to what the format string says it should do. Missing data, added comma,...

Note: in settings, you see the example lines working just fine, and that is using the same routine to generate the actual filename.

Just being curious, but it looks like you're using an older RMTV version - still does not explain the weird naming that you're seeing.
Can you give the beta a try?
